  • Missing passport report from the police.
  • MyKad/MyKid” of the applicant (National Identification Card).
  • A copy of the applicant’s birth certificate and a copy of “Both Parents’ MyKads” (if the applicant is under 18 years old).
  • A temporary identity card (JPN.KPPK09 or JPN.KPPK11) is required if the applicant does not have a MyKad.
